Due to the rapid spread of COVID-19, our practice is doing our part to ensure that the virus is contained and not spread to our patients. All new patients need to complete and return the new patient paperwork before they are seen by Dr. Bagheri. The paperwork can be found at the link below. Additionally, new patients must also submit copies of the front and back of both their ID and insurance card(s). All required documents (new patient paperwork and copies of ID and insurance cards) can be emailed to info@bitabagherimd.com. If you do not have access to email, your paperwork can be faxed to (949) 236-7900.
